New to Dogtown?

We are excited to meet you and your pup! Please submit the following Registration Form or you can download it and email it to us at We will contact you to setup an evaluation and a tour of our facilities.

On Evaluation day, it is very important to bring your dog(s) on a leash or slip lead with a collar/harness when you enter the lobby. The Evaluation includes a socialization and behavior assessment with your dog that takes 20-30 minutes with our trained evaluator. During this time, we will also show you around, address your questions and you can even watch the playroom cameras.

Evaluation includes:

  • Wellness check
  • Response to staff touch/interaction
  • Comfort level in a crate or suite which are used for rest, naptime, boarding, and feedings
  • Reaction in “open play” setting with other dogs of varying size, breed, and temperament
  • Overall social and behavior observations

Because dogs are a really big deal to us, we feel the Evaluation at Dogtown is fun for the whole family. Consider it an introduction to our open-play environment perfectly suited for your pup’s fitness and social life. In fact, don’t be surprised if your furry child meets his or her BFF while you’re here. The process is very easy and there is nothing to be nervous about at Dogtown – we just want to get to know your pup better!

PLEASE NOTEYour dog must be at least 16 weeks old with boosters and Bordetella vaccinations. Also, dogs older than 7 months must be spayed or neutered.

Evaluation Waiver

I, certify that my dog is in good health, has not been ill with any communicable diseases or parasites in the last 30 days, and has not harmed or shown aggressive or threatening behavior towards any person or any other dog. I also have read and understand and agree to the following:

1. Inherent risks of daycare, boarding, spa and grooming. I understand that Dogtown is an open-play environment and because of this there are inherent risks, which even when closely monitored, may result in the following:

a. Transfer of communicable parasites or an illness such as, but not limited to, the canine papilloma virus also known as “puppy warts,” or an upper respiratory illness like Kennel Cough, which can be caused by a contagious bacteria or virus.

b. Injuries, usually benign, such as broken nails, sore pads, puncture wounds, abrasions and cuts, particularly in shorter coated breeds, etc.

c. Behavioral problems.

2. Release. Dogtown and their team will not be liable for any health or behavioral problems that develop in my dog(s), and I hereby release them of any liability of any kind whatsoever arising from my dog(s) attendance and participation at Dogtown. I am solely responsible for any harm, including to any other dog(s), to the employees or invitees of Dogtown, or to the equipment, physical plant, or other property of Dogtown, caused by my dog(s) while my dog(s) is/are attending Dogtown.

3. Crate usage. I authorize my dog(s) to be placed in a crate during the evaluation process, boarding and/or daycare.

4. During my absence, Dogtown will be caring for my dog. In the event of an emergency, I authorized a veterinarian to administer medical treatment, and will be responsible for payment upon my return.

a. In case of emergency where I and my emergency contract on file are unreachable, I give permission to provide treatment while waiting to contact me or my emergency contact. I authorize treatment costs up to…

b. I agree that Dogtown is released from all liability related to transportation to and from veterinarian and treatment for sickness or emergency. This agreement will remain valid for all visits unless a new on is signed at my request.
5. I give Dogtown permission to transport my dog to a Veterinary Hospital in the event of an emergency or sickness and authorize treatment. If emergency is needed after regular office hours, my dog may be taken to an Emergency Veterinary Hospital.
